1.	Complete the New User Application and Service Agreement and Method of Payment forms.
2.	Print and Sign both forms.
3.	Fax the signed forms to (978) 524-8767
	or
	Mail the signed hardcopies of both forms with supporting licensing information to:
	LocatePLUS, Inc. 100 Cummings Center Suite 235M Beverly, MA 01915
You must also submit a copy of a state recognized license with your application.
Applications cannot be processed without these documents.
We must receive the application form with supporting licensing information within 10 business days after the forms have been faxed.
